Gynecomastia is the medical term for enlargement of the male breast and is a common problem. During puberty it is normal for boys to have some breast development. This is usually limited and resolves within 10 to 18 months. A variety of things such as medications, hormonal imbalances and genetics can influence the continued growth of breast tissue. However, in the majority of cases there is no apparent cause for this growth.
Like in the female breast, the male breast contains a variable combination of glandular and fatty tissue. The distribution and quantity of this tissue will determine the type treatment necessary. A detailed evaluation is first performed to exclude the above mentioned causes and if a patient is near puberty several months of observation may be requested to see if the growth resolves. In those patients that require treatment, the options will include ultrasonic liposuction, surgical removal of the gland and or excess skin, or a combination of these procedures.


In those patients whose skin quality is good and have predominantly fatty or diffusely glandular breasts, ultrasonic liposuction can be curative. When there is a focally dense area of breast tissue behind the areola, direct surgical excision with a small incision in the lower half of the areola is the treatment of choice. If there is also a significant amount of skin then skin removal to shape and tighten the breast is required. The goal of treatment is to remove a sufficient amount of breast tissue to minimize pain and recurrence but yet allow for a normal postoperative breast appearance.

Since our plastic surgery center was featured on the Real Housewives of New Jersey, an interesting thing has happened: more people seem to be thinking about undergoing cosmetic surgery themselves!
A recent study by the American Society of Plastic Surgeons confirms the correlation between reality television and an increase in procedures. The study looked at more than 2000 college women and showed that when people watch cosmetic surgery on reality television, it creates a more favorable attitude towards this type of surgery and a decreased fear of the risks associated with it.
Part of the reason is that these shows increase the average American’s knowledge of the procedures available. And seeing the consultation and surgery right from your living room somehow decreases the fear of actually having those procedures yourself.
As a plastic surgeon who takes great pride in helping my patients improve the way they look and feel, I only caution you about one thing: the ultimate responsibility of helping patients understand both the possible risks and the rewards of cosmetic procedures needs to remain in the hands of your plastic surgeon, not television.
When Real Houswives Theresa Guidice decided to share her experience of breast enhancement on national television, we were happy to be a part of the segment and to educate the public. But Theresa’s health and well being remained our top priority.

Patients often come for consultation regarding fullness in their abdominal mid section. This can be a result of a previous pregnancy, age or fluctuations in weight. Through hormonal influence, these factors will control the amount and distribution of body fat as well as skin and connective tissue laxity of the abdomen. Diet and exercise can help provide contour to these areas but only to a point. If you decide to further improve your appearance in this regard there will be a variety of surgical options available to you.
Abdominoplasty, or Tummy Tuck, is designed to remove the lax skin and fat of the abdomen and tighten the weakened abdominal wall. There are a variety of modifications of this procedure which are adapted to the patients specific problems and concerns. Some patients may only need skin removal while others may need fat removal and tightening of the abdominal muscles. Still others may only require muscle tightening.
Patients who present for consultation will undergo a thorough history and physical examination and have a treatment course specifically designed to address their unique problems. It is not uncommon for patients to come into the office expecting, for example, to have liposuction only to discover that their lax skin will become more lax if it is not tightened with a tummy tuck.
My role as the Plastic Surgeon is to outline and explain the options with their advantages and disadvantages so patients are well informed regarding the procedure that is right for them.

While women are more likely to choose some form of cosmetic intervention, the idea is catching on with men. According to the American Society of Plastic Surgeons, the top five male cosmetic surgical procedures in 2008 were nose reshaping; eyelid surgery; liposuction; gynecomastia or male breast reduction surgery; and hair transplantation. Over the past several weeks I have received numerous comments on my appearance on The Real Housewives of New Jersey and continue to be amazed at the number of people who have seen the episode. I am frequently asked what it was like to do the show and be with the housewives. I can honestly tell you that they were all very gracious and actually quite knowledgeable regarding breast augmentation. The consultation wasn’t nearly as hectic as it seemed and surgery went off without a hitch. Theresa was lovely and was one of our best patients. “The nose is the central part of the face,” explains the Cleveland Clinic-trained doctor who is also board certified in both plastic and general surgery. “If there are any deformities to it, people can become very self-conscious and that definitely impacts one’s life. I see it all the time in our ‘before’ and ‘after’ pictures. The ‘before’ picture oftentimes looks like a mug shot or a driver’s license photo. But in the ‘after’ photo, I usually see someone smiling and self confident. In that one photo, you can really see the change in self esteem. The goal of any surgical procedure, and especially that of eyelid surgery or Blepharoplasty, is to achieve a natural, well rested appearance without the stigmata of surgery. Sagging skin and wrinkles of the upper eyelid can give the appearance of fatigue and age well beyond one’s years. At The Robert Zubowski MD Center for Plastic & Reconstructive Surgery in Paramus, New Jersey, upper eyelid surgery can restore youthful volume and smooth contours that are so vital to a natural and youthful look. Under local anesthesia and sedation (or general anesthesia) an incision is made across the eyelid within the eyelid crease to remove excess skin and fat and improve youthful contour. Sutures are placed which are then removed within several days, leaving an incision which is practically undetectable.

The techniques for lower eyelid surgery are varied and today are tailored to the patient’s specific anatomical changes. We all age somewhat differently; some with excess fat deposits resulting in puffiness to the lower lids, some with skin redundancy and others with skin wrinkles or dark circles. Still others have a combination of the above.
Options for lower eyelid rejuvenation include removal or redistribution of fat deposits through concealed incisions within the eyelid, removal of skin redundancy by excising a small sliver of skin just below the eyelash margin, tightening lax muscle tissue and ironing out fine wrinkles with light laser or chemical peels. There are even non-surgical options for patients with minimal aging changes which include a combination of peels and fillers to tighten loose skin and fill tear trough depressions.
